My daughter was borh in 2011 in Manipal hospital, Bangalore and we have already got her Birth certificate made but in her Birth certificate the surname is not there. It is just read as "Swarnim" whereas rest of her documents carry Swarnim Tripathi (Tripathi is my husband's surname). We want to make correction and get the surname added to her name in the birth certificate. what all the documents would be required? What will be the format of request letter?
At the same time we need to correct my Husband's name in her Birth certificate as his middle name is not there in her birth certificate.
Can both be done at the same time? What are the documents we will need to get the corrections done.

Please help!
