I for myself and also for my wife want to apply for a passport.

I for myself and also for my wife want to apply for a passport. What is the documents required for the same. I am staying on a rental basis because of which I constantly change my address. For address proof is it ok to submit the copy of the agreement. It is only notarized by a notary with document registration number and not registered with registrar of societies. Or a bank statement will do in which my latest address appears.
