I am seeking an help from your end for obtaining a Death Certificate
for my uncle who expired on 19th December 2015 in Vashi, Sector 31,
during his visit for check up in Tata Memorial Center , He was
surviving from tongue cancer and was a resident of Guwahati, Assam ,
At the time of death he was accompanied by his wife alone and has been
cremented in Vashi,
As per the cremation authorities they stated that the receipt from
cremation ghat will be obtained after 7 days , and a person from the
cremation ghat assured that he would help us to get the document and
submit the same to the Municipality office for getting the death
certificate after 15 days, but unfortunately the same person is not
responding now, and all the legal claims from different government
office is held up for that. and it is very difficult for his nearest
family member to visit Mumbai from Guwahati(Assam) is really very
tough as because of the language barrier.
So I Chiranjib Chakraborty, nephew of the deceased is requesting you
to kindly provide some assistance to acquire the same from the
municipality,
We would be really helpful if you can provide at least the details of
the concerned authority through which we can collect the death
certificate as its a very tough time for the family in terms of
finance, as all the claims from LIC and other offices are pending from
past 2 months.
