I want to change my son's name & address mentioned in the birth

I want to change my son's name & address mentioned in the birth certificate which i got in Tuticorin.
He was born in August 2014 & he has valid Passport as well.

Can some one help me in providing details/process for applying changes in birth certificate and Passport.
