My elder daughter, name : PHOEBA NAVOMI ABRHAM,she was born on 0406.1996 at Pushpagiri Hospital, Thiruvalla. We had already collected original birth certificate from Municipality office at Thiruvalla. Unfortunately, we have lost the original in our working place in Mangalore ( Mangalapuram) in
Karnataka State. Now we need duplicate birth certificate of my daughter. What is the procedure to avail duplicate certificate. For your reference, we can submit the photo copy of the birth certificate. For issuing duplicate birth certificate how much fee we have to pay to Municipality.
