My son born at Jetpur (Dist. - Rajkot) on 2/5/1995 and the name

My son born at Jetpur (Dist. - Rajkot) on 2/5/1995 and the name written on his Birth Certificate is Shubham, Father's Name is Rajeshbhai Pratinbhai Desai and Mother's name is Nishaben. In all other documents including Aadhaar Card, PAN Card, Ration Card and Marksheets the names are written as Shubham Desai, Rajesh Desai and Nisha Desai respectively (which are correct). Now I want to correct the Names in his Birth Certificate.
Please let me know the procedure.
