Dear Sir/Madam,

I left job in Sup 2012 and planned to start business of Ball pens and gift products and wanted to register proprietorship company in Vasai Area, I met CA Bharat Patelia from Borivali through my friend's reference and given all documents.

I registered online for taking Professional Tax, then visited PT Officer with all documents required with form. PT Officer Mr. A.T. Ransing Nallasopara code NAL-PTO-C-001 asked me to Pay Rs. 500 to him, Rs. 200 for each clerk and Rs. 100 to each Peon if i want the PT number. I paid Total Rs. 1100 bribe as he told me to pay.

Than opened current account in South Indian Bank, Vasai Branch.

Than applied for VAT, Paid bribe to Mr. D. M. Gavit (Sales tax officer) (VAT-C-001 Registration, Nallasopara) Rs. 1500 and Rs. 200 to each clerk and peon, Total bribe paid Rs. 2000.

On 9th July i took add-on Name for my business on same VAT no. and added some products in list of commodities. For this Mr. D.M. Gavit took Rs. 1000 bribe from me.

Total bribe i paid Rs. 4100 for PT and VAT registration.
