There was no district name on my addressproof(bank passbook) application was rejecting

I was at RTO OFFICE Telangana Bolaram Rd, Chandragiri Colony, Lal Bahadur Colony, Trimulgherry, Secunderabad, Telangana 500015, India. for an vehicle ownership transfer from my wife name to my name .i have all my documents and filled the application online and everything was well enough to go to RTO.when i visited counter it was taking much time to process ,still i waited .when my turn came the officer has verified my application and rejected it saying mine address proof doesn't has district name .felt frustrated and returning home .one of the broker residing there asked me about my problem ,and said he can solve my issue and can do my work within 10 mins.that guy took my file and asked RS 600 as charges (100 for him and +500 to officer),i have seen that guy kept 500 note in my file.he just gave that file to officer and asked me to stay in the line for photograph.till return from the photo he iwas done with my application.And i have witnessed most of the people there following same process due to rejection with silly reasons.
