In the birth certificate Mothers name is shown as Jane Harish. Whereas

In the birth certificate Mothers name is shown as Jane Harish. Whereas all the documents are in the name of Jane Fernandes. Kindly advise whether we can approach the corporation office for correction as I want to apply for Passport.
My Date of Birth is 16/09/1997. My tenth certificate and PUC certificates are showing the name of my parents as follows:
Name of Father : Harish Sanil
Name of Mother: Jane Fernandes

Kindly advise.
