Hi, 1. when I applied my first baby birth certificate, I got the

Hi,
1. when I applied my first baby birth certificate, I got the problem (in mother name was missing some letters). After that I applied again with notary , letter from the hospital and my id proofs, and I got correct birth certificate.
2. Now also I got the same problem in my 2nd baby birth certificate.
In certificate mother name was mention " KARUMANCHI BALA SUNDARI",
but the correct name" KARUMANCHI BALASUNDARI" ( there is no space BALASUNDARI).

Now again I have to submit all the documents or only written request letter is enough .

Please give the suggestion for this one.
