I applied correction for name in pan card 4 months back. But it

• New PAN Card

I applied correction for name in pan card 4 months back. But it is not done yet and they ask for some Gazette certificate. And much effort i manage to get that gazette copy but now i want to know where should i send this gazette copy and is there anything else i need to submit with this gazette copy.
