I want to apply for my daughters passport who is a minor ( 6 months). my name on her birth certificate is Venecia Catul e Rebello. But my name on my passport and ration card is Venecia Veronica Victor Catul e Rebello. Now I have to make changes to my daughters birth certificate at Ponda Municipal Council, Goa by adding my two middle names. i wanted to know what are the formalities to reissue the birth certificate.
