Passport Annexure Form

I had applied for my son who is r5 months old in takkal category. It has be more then 45 days did not received the passport when personally visited the passport office got to know that one of the supporting annexure form is missing and need to submit again, I really don;t how the form got missed because all documents were thoroughly verified by the officer who had collected the application and fees. When asked for form he said need to get from XEROX shop outside. When approached the xerox shop he had demanded 500/- INR, After lot of agrument I give up and paid him the 500 RS for the form and submitted.
To surprise its not only me there are quite few people with same issue. Looks like some golmal with passport office staff and XEROX shop owner.
