Dear Sir,
I wish to get a new passport for my child below 5yrs. I hold the following documents.

a. Self PP issued at Trichy ( Tirunelveli rural address)
b. Wife PP issued at Chennai (Chennai address)
c. Ration card (inclusion my child name and my wife) issued at Chennai.
d.Child's birth certificate.
e. Pan card (On my name)
e. Pass Book (IOB) on my name.

Spouse's name is not endorsed either in my PP or wife's PP.

Please in addtion what are additional documents required to apply for a new PP for my child.
